IL-NET Presents… A National Webinar & Teleconference: Hospital to Home – A Collaborative Between Community Partners

The Hospital to Home (H2H) program is one example of how a Center for Independent Living (CIL) identified an existing program with the potential to be a service the health care industry would be interested in funding. The Independence Center (The IC) and UCHealth Memorial Hospital collaborated to facilitate successful transitions to home for patients with a complex or difficult discharge. Through H2H, The IC provides service assessment, coordination, and a network of local community-based organizations to provide support services that address social determinants of health for patients discharging from the hospital, so they can successfully thrive at home. Part I of this two-part training will start with the concept and vision of the H2H program; Part II will finish with the steps to set up a hospital to home program in your area.

Your Presenters

Patricia Yeager is the CEO of The Independence Center, a 30+-year-old organization serving the Pikes Peak area. The IC operates a traditional (skilled) home health agency as well as one of El Paso County’s largest providers of consumer-directed Home and Community Based Services (unskilled) aimed at keeping people with disabilities out of long-term nursing homes or other similar facilities. The IC employs 350 people between Home Health and the Independent Living Center. The IC has annual revenues of roughly $10 million. For nearly 40 years, Patricia has worked in the disability services and advocacy fields in West Virginia, Pennsylvania, California, and Colorado. She completed her PhD in Human Rehabilitation at the University of Northern Colorado in 2011 and has worked as a consultant on a variety of projects involving Independent Living-civil rights for people with disabilities. She holds a Master of Science in rehabilitation counseling from West Virginia University and a Bachelor of Arts in education from Marshall University.

Joseph Foecking PT, MHS, graduated from Ohio University with Bachelor of Science degrees in physical therapy and zoology. He also is a graduate of the University of Indianapolis and holds a master’s degree in health science. Joseph has practiced physical therapy in a variety of settings in many states since graduating in 1991. Currently, Joseph is the Director of Rehabilitation for UCHealth’s southern region. He oversees the rehabilitation services provided at both Memorial hospitals (Central and North), Grandview Hospital, Pikes Peak Regional Hospital, and six outpatient locations. Joseph is thrilled to be part of the board of directors for The Independence Center. With his background in physical therapy and rehabilitation, he believes it’s an excellent fit.

Indy Frazee started with The Independence Center in 2014 as their Controller. In 2018, she transitioned to the position of Director of Home Health where she oversees home health services, the CNA Training Program, the Hospital to Home Program, and the Veteran In Charge Program. Before joining The Independence Center, Indy served six years as the Director of Finance for Discover Goodwill. The first 10 years of Indy’s career was in public accounting and financial reporting for a large insurance company in Denver. She holds a Master of Business Administration from The University of Colorado at Colorado Springs and a Bachelor of Science in accounting from Colorado State University.

Amanda Strantz started with The Independence Center as the Care Transition Coordinator for the Hospital to Home Pilot Program in 2018. As the Care Transition Coordinator, Mandi provides intensive case management services to patients discharging from the hospital back into their homes. Before joining The Independence Center, Mandi was a child welfare caseworker with DHS and worked as an intake coordinator with The Resource Exchange, the local single entry point for long-term services and supports. She holds a Bachelor of Science in psychology from Southern Illinois University at Edwardsville.

The IL-NET National Training and Technical Assistance (T&TA) Center for Independent Living is operated by ILRU (Independent Living Research Utilization). The IL-NET T&TA Center provides training and technical assistance to centers for independent living and statewide independent living councils. ILRU arranges some training activities through the National Council on Independent Living (NCIL), peer-to-peer mentoring and youth program support through the Association of Programs for Rural Independent Living (APRIL), and online courses and document development through Utah State University Center for Persons with Disabilities.
